How can I replace this old tap? It's the main switch for hot water and I can't completely turn it off.

The current tap looks in rather good shape and quite new. Has it been replaced recently?
There shouldn't be any need to replace the tap. If it is not functioning, then the tap valve would likely just need replacing. The reason I've asked if the tap is new is that if it is and it has a new tap valve, then the seat in the fitting might need resurfacing as the valve is not making contact with it correctly. You could either attempt to reseat the tap or have a plumber come in and replace it. The tap body is soldered to the pipe, so you'll need a plumber to replace it.
