My dwarf lemon tree is sick.
Last year it had a dozen lemons and the perfume of the flowers was beautiful.
i have tried to save it using the spray in the photo however it hasn’t helped at all.

Your plant is suffering from citrus leaf miner as seen by the silvery track marks. Here's a helpful guide: How to get rid of citrus leaf miner. I'd suggest using Yates 200ml Success Ultra Insect Control on your plant. Note that the curled leaves will not return to their original form but will eventually drop and be replaced by new leaves. You might need to apply several cycles of the Success Ultra until this issue is under control.

Do I need to trim back the tree first?
I would treat the plant and let it drop the effected leaves naturally @menkap. If the leaf is entirely curled up, then you can remove it, as the curling will not reverse itself.
