How to fix a loose tap?

This tap is loose, like it's lost its bite. I have to turn it like 5 rounds before I can turn the water on or off. 

 

I've tried tightening with Alan key but it only fixes it for half a day and becomes loose again. 

 

What is the problem and what do I need to fix it?

Do I need a plumber or can it be done ourselves?

Welcome to the Bunnings Workshop community. It's sensational to have you join us, and thanks for sharing your question about your loose tap.

 

I believe that the spindle head has worn down and no amount of tightening of the grub screw will fix the issue. If you've done spindle replacements before, I suggest changing the spindle to a new one so that the handle will have something new to grab onto. If you've not done any plumbing before, I suggest engaging the services of licensed plumber to repair the tap for you.
