I would like to know what type of tap this is and how replace the washer to stop the leak.
please advise any kits that would apply to this tap.

You'll see good examples of how to remove your tap handle. In regards to changing the washer, I recommend looking at this guide - How to replace a tap washer.
Once the handle is removed, the next piece is the cover flange followed by the spindle itself. You then change the jumper valve to a new one and re-assemble the tap in reverse order. But if it is ceramic spindle, you'll need to replace it with a similar item. Please make sure to bring the old ceramic spindle with you to make sure you get the correct size.
