Can I use concrete mold? And how do I match the color concrete?

It will be difficult to match the concrete moulding unless you have the original moulding frame used on the surface of the concrete. The same issue comes up with colour matching as the original colour of your concrete surface has faded away, it would be impossible to match as the original colour has faded and worn away. If you are just after a patch, it would probably be possible to match the cut-out lines of the concrete but that would be as far as you would be able to go.
One other option is to clean the surface of the concrete driveway and paint it with a new surface colour.
